Girls Team Season Summary Form (entry/seeding & at-large application)
ATTENTION "APPLE" OWNERS! Some Apple products have issues with these Adobe interactive forms. These issues can be minimized, if not solved, but using the following steps:
- Download the form using "Firefox" (rather than the Apple default browser "Safari"). "Firefox is available free from Mozilla.
- Open the form in "Adobe Reader" (rather than the Apple default PDF reader "Preview"). "Adobe Reader" is avaialbe free from Adobe.
- Please save your completed "Season Summary Sheet" to your computer before you email it as an attachment to ccsplayoffs@cifccs.org.
